Introduction Stress is recognized as one of the key factors in the onset, exacerbation, and recurrence of mood episodes in bipolar disorder. This case study was conducted with the aim of implementing the nursing process based on Betty Neuman’s Systems Model in the care of a 25-year-old woman diagnosed with bipolar disorder type II.
Methods This study was carried out using a case study approach. The patient was purposively selected from among hospitalized patients in the female psychiatric ward of a university teaching hospital in Hamadan, Iran. Data were collected through semi-structured interviews with the patient and her mother (primary caregiver), direct observation, and review of medical records. A comprehensive assessment was performed according to Neuman’s Systems Model. Nursing diagnoses were formulated, and nursing interventions were implemented at three levels of prevention (primary, secondary, and tertiary). Care was provided continuously throughout one month of hospitalization, followed by two months of telephone follow-up after discharge.
Findings The main stressors identified included grandiose delusions, impulsive behaviors, conflict in the maternal role, recent divorce, cultural pressures from a rural community, and financial dependence on the family. The implementation of interventions based on Neuman’s Systems Model, alongside pharmacological treatment and multidisciplinary team collaboration, yielded positive outcomes. Grandiose delusions, verbal aggression, sleep duration, and social interactions showed significant improvement. The patient reported a greater sense of competence in performing her maternal role. Both the patient and her mother confirmed a marked reduction in stress levels, increased adherence to treatment, and overall improvement in functioning.
Conclusion The application of Betty Neuman’s Systems Model in the nursing process provides a comprehensive framework for identifying stressors and delivering holistic care. By emphasizing prevention at three levels and paying special attention to the patient’s perception of stressors, this model can significantly enhance the effectiveness of psychiatric nursing care for patients with bipolar disorder type II. It also contributes to improving quality of life and reducing recurrence. Therefore, this model can be recommended as a practical framework for education and implementation in psychiatric wards.
