Background and Objective: The follow-up program for discharged preterm infants is crucial to improving neonatal health and has faced challenges in the healthcare system. This study aimed to develop a follow-up program for discharged preterm infants.
Methods: This strategic study was conducted from April 2022 to February 2024 in five main stages at Isfahan University of Medical Sciences. 1) Qualitative interviews: Interviews with stakeholders were conducted to assess the needs and problems in following up preterm infants after discharge. 2) Delphi technique: Prioritization of problems identified in the previous stage was performed using expert opinions. 3) SWOT analysis: Based on examining strengths, opportunities, threats, and weaknesses in the current post-discharge follow-up situation, existing problems and capacities were identified. 4) Problem root cause analysis tree: Using this method, a deeper analysis of the dimensions identified from previous stages was performed, and a draft follow-up program was developed. 5) Validation of the draft program by an expert panel: Evaluation, revision, and final approval of the draft program were carried out at this stage.
Results: Four main categories were identified: weak care empowerment, lack of an effective follow-up program, lack of psychological support, and ineffective maternal education. In the Delphi technique, the first two categories were given the highest priority. SWOT analysis confirmed weaknesses in both maternal care and current follow-up. In the root cause analysis tree section, specific program objectives were identified, including care weakness, factors disrupting learning, physical-psychological stresses in mothers, lack of access to post-discharge services, and repeated visits to medical centers. In the expert panel section, the development of telemedicine for program implementation was recommended.
Conclusion: This program can be used by policymakers in the Ministry of Health to improve infant post-discharge follow-ups.
