fa «نامه سردبیری» انگ اجتماعی مرتبط با HIV: معضلی برای پیشگیری از گسترش HIV Editorial Letter” HIV-related Stigma: A Dilemma in the Prevention of HIV Spread مامایی Midwifery پژوهشي Research <div style="text-align: justify;">ابتلا به HIV/AIDS یک معضل جهانی است که سه دهه از ظهور آن می‌گذرد(1) اما هنوز در بین دانشمندان علوم ذیربط مسأله‌ای پیچیده و منحصربه فرد محسوب می‌شود. در حالی که HIV/AIDS از نظر پزشکی یک بیماری مزمن و کشنده تعریف می‌شود، از لحاظ اجتماعی به عنوان یک انگ اجتماعی تلقی شده که شأن افراد مبتلا را مورد آماج خود قرار می‌دهد. به این معنی که فرد مبتلا به HIV در کنار تحمل بار درد و استرس ناشی از بیماری و ترس از مرگ ناچار است احساسات منفی دیگری نظیر طرد، پیش داوری، تحقیر و انواع تبعیض‌ها را از سوی جامعه تحمل نماید. به طوری که به نظر می‌رسد پیامدها و مشکلات ناشی از انگ اجتماعی مرتبط با HIV از خود بیماری دردناک‌تر باشد(2). از آنجا که بیماری HIV با رفتارهای قابل اجتنابی همچون رابطه جنسی غیرایمن و سوء مصرف مواد مخدر در ارتباط است، افراد مبتلا غالباً مسئول بیماری خود شناخته می‌شوند(3) و خصوصاً عدم اطلاع رسانی صحیح رسانه‌ها موجب شده که این افراد به عنوان مجرم در جامعه شناخته شوند(4). ترس از قضاوت یا تبعیض از سوی دیگران می‌تواند روش نگریستن افراد مبتلا به خود و نحوه تطابق با وضعیت بیماری را عمیقاً تحت تأثیر قرار دهد. این تجربه ممکن است موجب شود افراد خود را مورد سرزنش قرار داده و سزاوار بدرفتاری بدانند(5) و حتی از حق درمان و استفاده از سایر مزایای اجتماعی دست بکشند. این امر با آثار مخربی بر پیشگیری، مراقبت و درمان HIV همراه است. انگ مرتبط با HIV با تعصبات فرهنگی و مسایل جنسیتی نیز در تعامل است. در سراسر جهان، وقتی زنی به HIV مبتلا می‌شود با برخوردهای تبعیض گرایانه‌تری نسبت به مردان مواجه می‌شود. از این رو بسیاری از زنانی که با HIV زندگی می‌کنند به دلیل ترس از طرد شدن، خشونت و حتی مرگ، وضعیت بیماری خود را از همسرانشان مخفی می‌کنند. این امر منجر به از دست رفتن فرصت پیشگیری از عفونت همسر می‌شود و همچنین امکان دسترسی به خدمات مراقبتی و درمانی مورد نیاز را از زنان سلب می‌کند(6). در واقع یک ارتباط چرخشی بین انگ اجتماعی و HIV وجود دارد؛ افرادی که انگ و تبعیض را تجربه می‌کنند به حاشیه رانده شده و نسبت به ابتلای HIV آسیب پذیرتر می‌شوند. از سوی دیگر، افرادی که به HIV مبتلا هستند بیش از دیگران مستعد تجربه انگ و تبعیض می‌باشند(7). سازمان جهانی بهداشت، ترس از انگ و تبعیض را به عنوان مهمترین مانع برای انجام آزمایش HIV، آشکار کردن وضعیت HIV و دریافت دارو معرفی کرده است. گزارش شده است که در 35 درصد کشورها، بیش از 50 درصد مردم دارای نگرش‌های منفی و تبعیض آمیز نسبت به مبتلایان به HIV هستند(8) و تقریبا از هر هشت نفری که به HIV مبتلا هستند، یک نفر به دلیل ترس از انگ و تبعیض از خدمات بهداشتی استفاده نمی‌کند. در سال 2016، 60 درصد کشورها گزارش کردند که نگرش‌های تبعیض آمیز کارکنان بهداشتی به معتادین تزریقی مانع از ارایه کافی خدمات پیشگیرانه HIV به این افراد می‌گردد(9). انگ و تبعیض نسبت به افرادی که با HIV زندگی می‌کنند در میان کارکنان خدمات بهداشتی در تمام سطوح شایع بوده و به اشکال مختلفی دیده می‌شود. کارکنان بهداشتی ممکن است تماس خود با مبتلایان به HIV و مراقبت از این بیماران را کاهش دهند، درمان آنها را به تعویق اندازند یا افراد مبتلا به HIV را از سایر بیماران ایزوله کنند. این در حالی است که عدم تمایل افراد به انجام آزمایش HIV موجب تأخیر در تشخیص بیماری، پیشرفت بیماری به مرحله AIDS و درمان نامؤثر آن شده و احتمال انتقال عفونت به دیگران و مرگ زودهنگام را افزایش می‌دهد(7). با وجود پیشرفت‌های زیادی که در زمینه شناخت HIV و روش‌های انتقال آن صورت گرفته اما تلاش در زمینه کاهش انگ و برچسب زدن بر این افراد کافی نبوده است(10،11). این در حالی است که برخورد مؤثر با انگ و تبعیض مرتبط با HIV که مانعی عمده در مسیر اقدام هماهنگ در سطوح جهانی، منطقه‌ای، ملی و اجتماعی است، یک الزام حقوق بشری می‌باشد. بنابراین به کارگیری برنامه‌های اختصاصی که بر حقوق بیماران مبتلا به HIV تأکید دارند راهی شناخته شده برای ریشه کنی انگ می‌باشد(12). از آنجا که انگ پدیده‌ای است که به طور اجتماعی ساخته می‌شود(2)، ضروری است که رویکردی همه جانبه برای مقابله با آن اتخاذ گردد. از این رو مقابله با انگ باید در سطوح مختلف فردی، اجتماعی، سازمانی، و سیاست گذاری مورد توجه قرار گیرد. در سطح فردی، بالا بردن آگاهی افراد در خصوص انتقال، مراقبت و پیشگیری از HIV و دسترسی به خدمات و حقوق قانونی ضروری است. همچنین اهمیت حمایت‌های اجتماعی از مبتلایان به HIV نباید نادیده گرفته شود. در برنامه‌های آموزشی کارکنان خدمات بهداشتی باید محرک‌های فرهنگی ایجاد انگ و تبعیض از قبیل ترس‌های شخصی از عفونت، تعصب نسبت به گروه‌های مستعد و آسیب پذیر و سوء برداشت یا عدم آگاهی در مورد انتقال HIV، درمان و پیشگیری از آن مورد توجه دقیق قرار گیرند. رسانه‌ها باید آگاهی مردم در مورد HIV و آثار آن بر زندگی افراد را افزایش دهند و با برجسته کردن این موضوع که افراد مبتلا به HIV نیز همچون سایر انسان‌ها از حقوق انسانی برخوردارند، با تغییر نگرش مردم با انگ و تبعیض مرتبط با HIV مقابله نمایند. همچنین در سطح سیاست‌گذاری لازم است تبعیض‌ها در استخدام، آموزش، خدمات بهداشتی و سایر حیطه‌ها برای افراد مبتلا به HIV رفع گردد. در واقع، ریشه کنی انگ و تبعیض که مانع بزرگی در راه دستیابی به خدمات درمانی و پیشگیرانه است، کلید پایان دادن به همه گیری HIV است.</div> <div style="text-align: justify;"></div> <div style="text-align: justify;"> <div>HIV/AIDS is a global health concern that spans three decades of its emergence(1). However, it remains a complex, unique issue among experts. While HIV/AIDS is medically recognized as a chronic, fatal disease, there is a social stigma attached to the disease, which targets the dignity of the patients. In addition to the burden of the disease, stress, and fear of death, an HIV-infected patient is faced with negative emotions, such as social rejection, prejudice, humiliation, and discrimination, so that HIV-related stigma seems to be more burdensome for the patients than the disease(2). Since HIV infection is associated with avoidable behaviors (e.g., unsafe sexual relations and substance abuse), the patients are often responsible for their own illness(3). On the same note, the failure of the media to properly inform people has led the populace to regard AIDS immoral(4). The fear of judgment or discrimination from others deeply affects the self-image of the patients with HIV infection, as well as their coping abilities. This experience may cause self-blame in the patients, who may, as a result, presume themselves to be deserving of maltreatment(5). They may even give up their rights to treat and receive other social benefits, which is associated with devastating effects on HIV prevention, care, and treatment. HIV-related stigma also interacts with cultural prejudices and gender issues. Globally, when a woman is infected with HIV, her status subjects her to more discriminatory treatment compared to a man. Therefore, many women living with HIV tend to hide their status from their spouses due to the fear of rejection, violence, and even death. This leads to the loss of the opportunity to prevent the infection of the spouse, and also deprives women of access to the necessary care and treatment(6). There is a cyclical relationship between stigma and HIV; the individuals who experience stigma and discrimination due to HIV infection are marginalized, thereby becoming more vulnerable to HIV. On the other hand, patients with HIV are more prone to discrimination and stigma than others(7). The World Health Organization has identified the fear of stigma and discrimination as the main barriers to HIV testing, HIV status disclosure, and access to HIV treatment. It is reported that in 35% of countries, more than 50% of the population have discriminatory attitudes toward HIV patients(8), and about one out of eight peopleinfected with HIV does not use health services due to the fear of discrimination and social stigma.</div> <img align="left" alt="Text Box: 4" height="43" hspace="12" src="file:///C:/Users/LOCALU~1/AppData/Local/Temp/msohtmlclip1/01/clip_image001.png" vspace="5" width="168" >In 2016, 60% of the countries across the world reported the discriminatory attitude of healthcare providers toward intravenous drug users, which in turn discourages them to provide adequate services for HIV prevention to these patients(9). HIV-related stigma and discrimination have been frequently observed in healthcare professionals at all levels, and the attitudes are manifested in a variety of manners. Healthcare providers may minimize their contact with HIV, neglect their proper care, delay their treatment, and isolate these individuals from other patients. However, the unwillingness of people to receive HIV testing delays the diagnosis of the disease, leading to its progression to the stage of AIDS and ineffective treatment, thereby increasing the risk of infection transmission and untimely death(7). Despite the advancement in the detection of HIV and its transmission, there have not been sufficient effort to reduce the social stigma and labeling attached to this disease(10,11). Management of HIV-related stigma and discrimination is a major impediment to the coordinated action at global, regional, national, and social levels, as well as a human rights obligation. Implementing specific programs that emphasize on the rights of HIV patients is a well-known way of eradicating HIV-related stigma(12). Stigma is a social construction(2), and it is imperative to adopt a comprehensive approach to deal with it. Therefore, tackling with stigma should be taken into consideration at the individual, social, organizational, and policy-making levels. At the individual level, raising public awareness regarding HIV transmission, care, and prevention, while ensuring access to proper services and legal rights is absolutely essential. Another influential factor in this regard is providing social support to HIV patients. In the educational programs for healthcare providers, the cultural stimuli for the establishment of HIV-related stigma and discrimination (e.g., fear of infection, prejudice toward vulnerable groups, and misconceptions/lack of knowledge regarding HIV transmission, treatment, and prevention) should be considered. Furthermore, the media should raise public awareness regarding HIV and its effects on the lives of people, thereby tackling HIV-related stigma through changing the false attitudes and emphasizing on the fact that patients with HIV have the same human rights as others. At the policy-making level, factors such as discrimination in employment, education level, health service provision, and other needs of HIV patients must be addressed.
