نقش وضعیت همودینامیک و اکسیژن خون در بروز زخم فشاری در بیماران تحت تهویه مکانیکی
The Role of Hemodynamic Status and the Level of Oxygenation in the Development of Pressure Ulcer in Mechanically Ventilated Patients

<p> <strong> زمینه و هدف: </strong>علیرغم پیشرفت های سیستم بهداشتی-درمانی و هزینه های روزافزون پیشگیری از زخم فشاری، تعداد مبتلایان به زخم فشاری در بیماران بخش های ویژه رو به افزایش است. پژوهش حاضر با هدف تعیین ارتباط وضعیت همودینامیک و اکسیژن خون با بروز زخم فشاری در بیماران تحت تهویه مکانیکی در بخش های مراقبت ویژه صورت گرفت.<strong></strong> </p><p> <strong> روش بررسی: </strong>این مطالعه همبستگی از نوع طولی بود که ارتباط بروز زخم فشاری در طول مدت بستری را با سایرمتغیرهای اصلی پژوهش مورد بررسی قرار داد. جامعه پژوهش را بیماران تحت تهویه مکانیکی بستری در بخش های مراقبت ویژه تشکیل دادند. 120 بیمار با روش نمونه گیری مستمر انتخاب شدند. نمونه گیری از بهمن 1390 تا مرداد1391 ادامه داشت. ابزار گردآوری داده ها شامل اطلاعات جمعیت شناختی، ابزار برادن، فرم اطلاعات بالینی در خصوص وضعیت همودینامیک و اکسیژن خون و چک لیست مشاهده روزانه پوست بیمار بود. داده ها با استفاده از نرم افزار SPSS نسخه 17 و آزمون های تی مستقل، کای اسکویر و آنالیز رگرسیون مورد تجزیه و تحلیل قرار گرفتند. </p><p> <strong> یافته ها: </strong><strong></strong>در 8/35 درصد از بیماران زخم فشاری درجه یک ایجاد شد. بر اساس نتایج آنالیز رگرسیون لجستیک بین متغیرهای مربوط به وضعیت همودینامیک و اکسیژن خون با ایجاد زخم فشاری ارتباط معناداری دیده نشد. با اینحال طول مدت تهویه مکانیکی، سن و سطح هوشیاری بیشترین سهم را در ایجاد زخم فشاری داشتند(001 . 0 P< ). </p><strong>نتیجه گیری کلی: </strong>بیماران تحت تهویه مکانیکی از شانس نسبتا بالایی برای بروز زخم فشاری برخوردار هستند و لازم است پرستاران مراقبت کننده از این بیماران اقدامات پیشگیری را در مورد این بیماران مورد توجه قرار دهند. بررسی ارتباط سایر متغیرهای مداخله گر در این رابطه پیشنهاد می شود
<p> <strong>Background and Aim: </strong>Despite of developments in healthcare systems and ongoing investments to prevent pressure ulcers, it still remains as a major problem particularly in critical care units . This study aimed to investigate the relationship between the level of oxygenation and hemodynamic status in the occurrence of pressure ulcer in mechanically ventilated patients. </p><p> <strong>Material& Methods: </strong>It was a correlational and longitudinal study. The study population included 120 mechanically ventilated patients admitted to critical care units of one of the hospitals affiliated to Tehran U niversity of Medical Sciences in Tehran-Iran. The subjects were selected through consecutive sampling method from February 2012 to August 2012. Hemodynamic and oxygenation data were collected using the patients’ records and observation sheets. Patients’ skin was daily assessed for developing pressure ulcers. Pressure sore risk factors were assessed by Braden Scale. Collected data were imported into SPSS-PC (v.17) and analyzed using independent t-test, chi-square and logistic regression. </p><p> <strong>Results: </strong>pressure ulcers of stage I were developed in 35.8% of the subjects. There was no significant linear correlation between hemodynamic and oxygenation variables and the development of pressure ulcers. However there was a significant relationship between the development of pressure ulcers and the duration of mechanical ventilation, patients’ age and the level of consciousness ( P<0.001) . </p><p> <strong>Conclusions: </strong>Patients under mechanical ventilation have more chance for developing pressure ulcers and nurses should apply preventing masseurs as soon as possible for such patients. Investigating the role of other variables in the development of pressure ulcers in these patients is recommended.</p><p> Received: 14 Apr 2013 </p><p> Accepted: 6 Jun 2013 </p><p> </p>
