تأثیر آموزش به شیوه سخنرانی و چندبعدی بر بهبود زخم پای دیابتی و پایبندی بیماران به توصیه های مراقبتی
The Effects of Lecture and Multimodal Methods of Teaching on Healing Rate of Diabetic Foot Ulcer and Patients' Compliance with Care Recommendations

<p> <strong> زمینه و هدف: </strong>گروهی از بیماران مبتلا به دیابت از توصیههای مراقبت از پا پیروی نمیکنند. شیوه آموزش ممکن است از عوامل تاثیرگذار بر میزان پیروی از توصیه های مراقبتی و بهبود زخم باشد. از این رو، مطالعه حاضر با هدف مقایسه تاثیر دو شیوه آموزشی سخنرانی و چندبعدی بر پایبندی به توصیههای مراقبتی و بهبودی زخم پای دیابتی در سال 1390 در کاشان انجام شد. </p><p> <strong> روش </strong><strong><i /></strong><strong>بررسی </strong><strong>: </strong>مطالعه به صورت کارآزمایی تصادفی با گروه کنترل، در سه گروه 20نفره (آموزش به شیوه سخنرانی، آموزش به شیوه چند بعدی، و گروه کنترل) مبتلا به زخم پای دیابتی، مراجعه کننده به بیمارستان بهشتی کاشان انجام شد. هر یک از گروههای مداخله، با محتوای مشابه آموزش دیدند. پس از 3 ماه مساحت و عمق زخمهای سه گروه (از طریق بررسی زخم) و پایبندی به توصیههای مراقبتی (به کمک یک چک لیست مراقبت روزانه) مقایسه شد. تجزیه و تحلیل دادهها با آمار توصیفی، آزمونهای T-test ، paired t test ، آنالیز واریانس، ANCOVA و مجذور کای با استفاده از SPSS نسخه 5/11انجام شد. </p><p> <strong> یافتهها: </strong>پس از مداخله، مساحت و نیز عمق زخم در هر سه گروه نسبت به قبل از مداخله بطور معنیداری کاهش یافت (05/0> P ) اما میزان این کاهش در گروههای آموزش دیده و بویژه در گروه آموزش چندبعدی بیشتر بود. میانگین نمره پایبندی به مراقبت از پا در گروه سخنرانی 3/9 ± 4/66 و در گروه چندبعدی 4/7 ± 6/79 بود (009/0= <i>P </i>). </p><p> <strong> نتیجه </strong><strong><i> </i></strong><strong>گیری کلی: </strong>پایبندی به برنامه مراقبتی و نیز بهبود زخم پا در گروههای آموزش دیده بیش از گروه کنترل بود. این آثار بویژه در گروه آموزش چند بعدی بیشتر بود. توصیه میشود تا برنامههای آموزشی چندبعدی به بیماران دیابتی در نظام سلامت تلفیق شود تا بیماران در مراقبت از زخمهای پا توانمند شوند. همچنین، تکرار مطالعه با درنظر گرفتن سایر عوامل موثر بر بهبود زخم پیشنهاد میشود. </p><p> <strong> </strong></p>
<p> <strong>Background and Aim: </strong>Some patients with diabetes do not follow the foot care recommendations. Methods of patient education may affect the rate of compliance and ulcer healing. The present study aimed to compare the effects of teaching by lecture and multimodal method on compliance with foot care recommendations and healing of diabetic foot ulcers in kashan city, during 2011. </p><p> <strong>Material &Methods </strong><strong>: </strong>A controlled clinical trial was conducted on three groups (n=60) (including teaching by lecture, teaching by multimodal method, and control group) of diabetic patients with foot ulcers who had been referred to the Kashan's Beheshti hospital. The two intervention groups were taught by similar content. After three months, the surface and the depth of ulcers and the patients’ rate of compliance with recommendations were evaluated (by observing the ulcers and using a daily foot care checklist). Data analysis was done by t-test, paired t test, ANOVA, ANCOVA and Chi-square using SPSS-PC (v. 11.5). (Trial Registration Number: IRCT138901143618N1) </p><p> <strong>Results </strong>: Ulcers surface and depth were significantly decreased in all groups after the intervention (p<0.05) . However the decrease was more prominent in educated groups and especially in multimodal group. The mean score for compliance with foot care recommendations was 66.4±9.3 in lecture group and 79.6±7.4 in the multimodal group (p=0.009). </p><p> <strong>Conclusion </strong>: It is recommended that multimodal patient teaching be integrated in the health care system to empower the diabetic patients to manage their own foot ulcers. Repeating the study considering other factors affecting the healing of foot ulcers is also suggested.</p><p> Received: 21 May 2012 </p><p> Accepted: 5 Aug 2012 </p><p> </p>
