Background & Aims: Perimenopause period usually begins in the middle to late fifth decade of life and is often silent and uneventful. During this period, the consequences of systemic estrogen reduction appear. Irregularity of the menstrual pattern, vasomotor instability, mood changes, sleep disorders and muscle pains are among these consequences. One of the most important complications of these changes is the decline in the quality of life of women around menopause. For some women, the reproductive ability is one of the main sources of identity and self-confidence; Therefore, the idea of declining fertility may cause severe distress, Furthermore, regardless of the effect of loss of fertility, the distress of loss of youth experienced by menopause may be ambiguous but disturbing. although stress is a common and sometimes necessary thing in life, but if its level is higher than the desired level, it is considered abnormal. One of the important issues in this regard is the effect of stress on sexual performance. Therefore, one of the problems of women's transition to menopause is sexual dysfunction, which often begins at the age of 45. Therefore, stress management training can be accepted as a key continuous strategy to modify the way of life and deal with ineffective thoughts in a logical way to deal with emotions. Virtual education on stress management is the first source of information chosen by women due to its availability, quick response, and attractive ways of presenting content in many situations. Therefore, this research was conducted with the aim of determining the impact of the virtual stress management training on the sexual performance of perimenopausal women in Shahriar City.
Materials and methods: This clinical trial study of a before and after the group was conducted on 50 women aged 40 to 50 in Shahryar City in 2022. The studied population was all healthy women around menopause in Shahriar City. Inclusion criteria in this study include women with literacy who are able to use a smartphone, live with their husbands, have active sex, and do not use any hormonal or psychoactive drugs. Also, these participants experiences menstruated at least once in the last three months and must not experience any tension during this period. In this research, sampling was done continuously until reaching the quorum of the sample size at Sohrab Ali Bakshi Comprehensive Health Services Center. Stress management training for 5 groups of 10 people in the form of 8 training sessions including the first session, understanding the issues related to peri-menopause including sleep disorders, hot flashes, osteoporosis, etc., helping to create hope and empathetic relationship between the participants and the researcher. The second session was a review of the previous session, explaining the continuation of the topic of menopause, stress, the definition of stress and its management: understanding other issues and problems of menopause, understanding the meaning of stress, and the ability to perform behavioral and four-step stress management techniques. The third session includes a review of the previous sessions, stages of normal sexual activity, awareness of the effects of stress on menopause, awareness of the possible consequences of stress on health and sexual performance in menopause: familiarizing the participants with the stages of natural sexual activity and familiarizing them with common sexual complaints during menopause and their description. The fourth session reviews the previous sessions, identifying ineffective and wrong thoughts and beliefs about sexual activity in menopause, identifying rational and irrational self-talk: getting to know false beliefs about sexual activity during menopause, the ability to recognize logical and illogical self-talk and distinguishing between these there were two.The fifth session includes reviewing the previous sessions, understanding the relationship between thoughts, feelings, emotions and physical sensations, recognizing the types of negative thoughts and cognitive distortions, practicing identifying negative thoughts, replacing logical thoughts and performing exercises to replace logical thoughts with distorted thoughts. : The ability to recognize negative thoughts and replace them with positive thoughts, the ability to regulate uncontrollable emotions and feelings, the ability to differentiate thoughts from feelings. Sixth session review of previous sessions, performing muscle relaxation exercises for 6 muscle groups, learning the process and performing diaphragmatic breathing exercises, visualization in order to obtain gradual muscle relaxation, performing exercises combining diaphragmatic breathing with visualization and gradual muscle relaxation with location visualization Special: The ability to correctly perform muscle relaxation exercises and diaphragmatic breathing was according to the training
given. The seventh and eighth sessions were two interactive sessions with the presence of all members after coordinating their simultaneous virtual presence and discussing the assignments given in the previous sessions and solving the problems and ambiguities of the members. Each session was presented with a 30-minute clip, 2 times a week for 4 weeks, virtually and online in the WhatsApp social network environment. In order to ensure participation, participants were asked questions and answers on their personal pages. Questionnaires of individual social characteristics and a standard questionnaire of sexual performance before the intervention and 4 and 8 weeks after the intervention were completed through self-reporting in virtual form and sent to the researcher through the WhatsApp social network or e-mail. Data were analyzed using paired t-test and ANOVA.
Findings: The average age of participating women was 44.77 years with a standard deviation of 3.07 years. 56% of them had below diploma education, most of them were housewives (64%) and the ethnicity of half of them was Persian. Most of them (98%) were residents of the city. The mean and standard deviation of the total sexual function score before the intervention was 19.70 ± 2.22 and immediately after the intervention was 0.86 ± 52.4. Sexual performance in weeks 4 and 8 had statistically significant differences except for irritability and orgasm in other areas (P<0.001).
Conclusion: The virtual training of stress management improved the areas of desire, motivation, satisfaction, pain and the total score of sexual performance in
perimenopausal women. Virtual training did not improve irritability and orgasm in perimenopausal women. Using the type of counseling is an easy, accessible and low-cost method as one of the useful and effective methods in the sexual performance of perimenopausal women.
