تأثیر فشردن قفسه سینه در زمان بازدم قبل از ساکشن داخل تراشه بر تبادل گازهای خون شریانی بیماران تحت تهویه مکانیکی

<p align="right"> <strong> زمینه و هدف: </strong>یکی از معمولترین روشهای تمیز کردن راه هوایی بیماران دارای لوله داخل تراشه و تحت تهویه مکانیکی انجام ساکشن میباشد. به منظور تسهیل خروج ترشحات راههای هوایی و در نتیجه بهبود تبادلات گازی میتوان از روشهای فیزیوتراپی قفسه سینه نظیر فشردن قفسه سینه در زمان بازدم قبل از ساکشن داخل تراشه استفاده نمود. هدف پژوهش حاضر تعیین تأثیر فشردن قفسه سینه در زمان بازدم قبل از ساکشن داخل تراشه بر تبادل گازهای خون شریانی بیماران تحت تهویه مکانیکی انجام شد. </p><p align="right"> <strong> روش بررسی: </strong>این پژوهش یک مطالعه کارآزمایی بالینی با طرح متقاطع بود. واحدهای پژوهش شامل 70 بیمار تحت تهویه مکانیکی (35 زن، 35 مرد) بودند که در بخشهای مراقبتهای ویژه(داخلی و جراحی) و اورژانس مرکز آموزشی و درمانی حضرت رسول اکرم (ص) تهران بستری شده بودند. واحدهای پژوهش به روش نمونهگیری مستمر انتخاب شدند. بر روی هر کدام از بیماران دو مداخله ساکشن داخل تراشه با و بدون فشردن قفسه سینه در زمان بازدم انجام شد و بین دو مداخله حداقل 3 ساعت فاصله وجود داشت. تکنیک فشردن قفسه سینه در زمان بازدم به مدت 5 دقیقه قبل از ساکشن داخل تراشه انجام شد. گازهای خون شریانی 5 دقیقه قبل از ساکشن داخل تراشه و 25 دقیقه بعد از آن اندازهگیری شد. ابزار گردآوری دادهها یک برگه ثبت اطلاعات بود. دادهها با استفاده از آزمون ویلکاکسون و آزمون تی زوج توسط نرمافزار SPSS تجزیه و تحلیل شدند. </p><p align="right"> <strong> یافته ها: </strong>در هر دو روش (ساکشن داخل تراشه با و بدون فشردن قفسه سینه در زمان بازدم) بین متغیرهای"نسبت فشار اکسیژن شریانی به درصد اکسیژن دمی،" فشار دی اکسید کربن شریانی و درصد اشباع اکسیژن شریانی قبل و بعد از ساکشن داخل تراشه تفاوت معنیدار وجود داشت(000/0 <i>P </i>= ). علاوه بر این در مقایسه دو روش مذکور نیز بین متغیرهای فوقالذکر تفاوت معنیدار وجود داشت (به ترتیب 000/0 <i>P </i>= ، 048/0 <i>P </i>= و000/0 <i>P </i>= ). </p><p align="right"> <strong> نتیجهگیری: </strong>نتایج حاکی از آن بود که انجام ساکشن داخل تراشه با فشردن قفسه سینه در زمان بازدم به صورت معنیداری بیشتر از انجام ساکشن داخل تراشه به تنهایی موجب بهبود تبادل گازهای خون شریانی بیماران تحت تهویه مکانیکی شد؛ لذا پیشنهاد میگردد که در بیماران تحت تهویه مکانیکی قبل از ساکشن داخل تراشه از تکنیک فشردن قفسه سینه در زمان بازدم استفاده میشود. </p>
<p> <strong> Background & Aim </strong><strong>: </strong>Endotracheal suctioning is one of the most frequently used methods for airway clearance in patients under mechanical ventilation. Chest physiotherapy techniques such as expiratory rib cage compression before endotracheal suctioning can be used as a means to facilitate mobilizing and removing airway secretions and improving alveolar ventilation. This study was carried out to determine the effects of expiratory rib cage compression before endotracheal suctioning on arterial blood gases in patients mechanically ventilated patients. </p><p> <strong> Material & Method </strong><strong>: </strong>This research was a clinical trial study with a crossover design. The subjects consisted of 70 (35 females, 35 males) mechanically ventilated patients who were admitted to medical and surgical intensive care units and emergency department of Hazrat-Rasool Hospital in Tehran. Subjects were selected with consecutive sampling. The patients received endotracheal suctioning with and without rib cage compression, with a minimum 3-hours interval between the two interventions. Expiratory rib cage compression was performed for 5 minutes before endotracheal suctioning. Arterial blood gases were measured 5 minutes before and 25 minutes after endotracheal suctioning. Data were recorded on the data recording sheet. Data were analyzed using Wilcoxon and paired t-tests. </p><p> <strong> Results: </strong>There were a statistically significant differences in the ratio of arterial partial pressure of oxygen to fraction of inspired oxygen (Pao2/Fio2), Paco2, and arterial oxygen saturation (Sao2) between before and after endotracheal suctioning in both methods (endotracheal suctioning with and without rib cage compression) (p=0.000). Moreover, there were statistically significant differences in the ratio of arterial partial pressure of oxygen to fraction of inspired oxygen (P=0.000), Paco2 (P=0.048), and arterial oxygen saturation (P=0.000) between the two methods. </p><p> <strong> Conclusion: </strong>Findings showed that expiratory rib cage compression before endotracheal suctioning improves arterial blood gases exchanges in patients under mechanical ventilation, so, performing expiratory rib cage compression before endotracheal suctioning in these patients is recommended. </p>
